It begins with your scanner. It has to be set for a black and white image, or even a bitmap scan.
But, if not, just take the scanned image and convert it to a greyscale mode, and then to a bitmap mode. Play with the threshold controls to see which parts you'd like to go to white and which you'd like to go to black.
